- To define the differences between medical transfer of care and psychosocial transition to the adult medical system.
- To review the data strongly supporting that structured transition programs improve outcomes.
- To understand the vital components of a transition program.
- To share how different hospital systems may need varied approaches, and how we can address these challenges.
